What to Bring
To ensure a smooth check-in process, please bring the following items to every visit:
- Health insurance card
- Photo ID (e.g., driver’s license or passport)
- List of current medications, including supplements and over-the-counter medications
- Any relevant medical records or test results, if not already in our system
Need a Referral?
If your provider recommends care from a specialist, we’ll take care of the coordination. Our referral team works closely with Community Care Physicians’ network of trusted specialists, all of whom share the same electronic health records. This ensures your care is well-connected, with real-time access to your health information across providers.
Prefer to see a specialist outside of our network? We’re happy to accommodate your choice and facilitate the referral process.
Feeling Sick? Please Call First
If you are experiencing any symptoms of illness—such as fever, cough, congestion, sore throat, or stomach issues—please call our office before coming in. You may need additional instructions to help keep you, our staff, and other patients safe.

Location: You’ll find us conveniently located in Troy, NY, at the Rensselaer Technology Park. Free parking is available onsite.
